Starting in 1898, physicians and midwives were required to report every birth they attended to the county clerk each quarter. If no one attended the birth, the parents had to register the birth to the local county clerk. This changed in 1905 when the state decided to further regulate the reporting of vital statistics and required that a standard birth certificate must be filed with the newly created State Bureau of Vital Statistics.

Every birth record in the Utah State Archives is assigned a number and contains the personal information described above, along with the informant's name who reported the birth.
