Utah vital records are maintained by the Office of Vital Records and Statistics (http://health.utah.gov/vitalrecords/). Utah birth records and Utah death records are available since 1905 and are maintained by the office for 100 years for birth records and 50 years for death records. Older birth records older are kept with the Division of Archives & Records Service (http://archives.utah.gov/research/guides/birth.htm). Statewide birth and death registration began in 1898, though some cities registered deaths prior to that date. Birth and death records are available for a fee and with proper identification to immediate family members or to persons with written permission from the immediate family. Payment is by personal check or money order made payable to Vital Records.
