The Douglas County Health Department, where qualified individuals can apply for birth and death certificates, is at 603 NW 12th Ave. All pertinent information is provided on the Department website, as well as linking to its parent organization, the Missouri Department of Health and Senior Services, which also provides vital records for all counties at the state level.
