All vital records are also maintained at the state level. Birth records can be ordered at the Utah Department of Health for $18.00 for the first copy and $8.00 for each additional copy ordered at the same time. All vital records can be ordered through VitalChek for $38.95 plus shipping fees. Statewide birth and death registration began in 1898, though some cities registered deaths prior to that date.
