Requests for forms, information, and copies of records after six months are directed to the state (http://public.health.oregon.gov/BirthDeathCertificates/GetVitalRecords/Pages/index.aspx) where the fee is $20.00 for the first certified copy; additional copies are $15.00 each. Requests for copies of records can also be directed to VitalChek (http://www.vitalchek.com/). The non-refundable fee for one copy through VitalChek is $32.50, which includes a $12.50 processing fee. Copies of marriage and divorce records are also available online at VitalChek.
